Wellness Incentive Program
Welcome to the Arthrex Wellness Program 2020
The Arthrex Wellness Program rewards you for making healthy choices by reducing your medical insurance premium. Arthrex automatically pays 70% of the employee premium for health insurance for all benefit eligible full-time employees electing coverage. The remaining 30% can be earned by completing specific incentives provided through the Arthrex Wellness Program. Premium incentives are not provided for spouses or dependents.
EARNING INCENTIVES
Employees can view available incentives and monitor progress through their personal wellness portal at SimplyWell.com or through the SimplyWell app. See instructions below on creating and accessing your wellness portal and app.

INCENTIVE DEADLINE – OCTOBER 31
All incentives must be completed by October 31 to receive the premium discount the following calendar year. Based on the total incentives earned, Arthrex will pay 70%, 85% or 100% of your employee insurance premium.
VIEWING/TRACKING INCENTIVES
To view, monitor and ensure completion of incentives, all participating employees must create a SimplyWell a user account. Employees must also submit a completed Wellness Verification Form annually to SimplyWell. This form includes the results of your wellness labs and your preventive screening dates. Forms are available here and in your SimplyWell portal. It is the responsibility of the employee, and not the medical provider, to upload the completed form to SimplyWell prior to the October 31 deadline. Please allow approximately 20 business days from the date of the form upload to receive credit and view lab results in your wellness portal.
NEW HIRES
If hired on or after May 1, 2020, you will automatically earn 100% of the medical premium for 2021. To maintain this coverage, you will be required to complete the incentive program requirements in 2021 for the following year’s coverage.
CREATING YOUR WELLNESS PORTAL THROUGH SIMPLYWELL
If you have already created an account through the SimplyWell portal, you do not need to re-register. Follow the instructions below to register if you are a first-time user. Please allow approximately 2 weeks from your orientation date prior to creating an account for your wellness portal.
- Visit connect.simplywell.com
- Select REGISTER
- Enter requested information
- Identifier: use your Arthrex network ID
- Registration Code: Arthrex Wellness
- Follow the prompts to complete registration
Notes:
If you have an Arthrex Network ID less than 4 characters, you will need to use: Arthrex Network ID + 1234 (example: abw1234)
Spouse Identifier: “SP-“ proceeds the Arthrex Network employee ID
