  • Where is the Wellness Verification Form located?

    The form is located HERE and in your SimplyWell portal under Profile/Resources/Forms and Documents. If completing incentives outside of an Arthrex Medical Center, please print and take the form to your provider.

  • How do I submit my completed Wellness Verification Form?

    Take a photo of the form or scan and save the form in your electronic files. Log into SimplyWell, Select Profile and Form Upload. Select Physician Form and Attach File. Allow 20 days from the date of upload for your wellness portal to update. Completed forms can also be faxed to SimplyWell at 855.292.8662 or mailed to Screening Services Department, 10670 N. Central Expwy., Suite 250, Dallas, TX 75231.

  • Why may some points be missing from my SimplyWell portal?

    Please allow approximately 20 business days from the date of the form upload to receive screening credit and view lab results in your wellness portal. If preventive health screenings were completed after you submitted your form, you will receive completion credit once the medical claim is processed; however, it may take up to three months before your wellness portal reflects completion. Employees completing any preventive health screenings after July 1 are strongly encouraged to upload a Wellness Verification Form with dates of preventive screenings for quicker portal updates.

  • How do I check my progress?

    Log into your account at Simplywell.com or download the SimplyWell app.

  • What values are checked during my wellness labs?

    The following labs/biometrics are part of the wellness incentive and must be measured as part of your wellness lab work: total cholesterol, HDL, LDL, triglycerides, blood pressure, height, weight, hemoglobin A1c, and nicotine. Your physician may recommend additional labs based on your personal needs and history.

  • Do I have to go to an Arthrex Medical Center (AMC) for my physical and labs?

    Access to the Arthrex Medical Centers is an incredible benefit available to all employees, spouses and dependents on day one of employment. The clinics are convenient options to schedule your annual physical and wellness labs. To schedule an appointment with one of the AMC’s, please call 239.591.1488. If you choose an outside physician for your lab work and annual physical, select an in-network provider and lab. Provide your physician with the Wellness Verification Form located in your SimplyWell portal.

  • What is the member health assessment (MHA)?

    The MHA is a health questionnaire available to complete in your SimplyWell portal. It is not part of the wellness incentive program; however, it is a terrific optional tool to complete if you are interested in learning more about your potential health risks and ways to improve.

  • If I am a new employee, when can I create my SimplyWell account?

    Please allow approximately 2 weeks from your hire date prior to creating an account for your wellness portal.

  • What if I am pregnant?

    You can choose to complete the wellness incentives as outlined above or you can submit a completed Pregnancy Alternative Request Form available HERE and in your SimplyWell portal under Profile – Resources – Forms & Documents. Once verified, you will earn 100% premium coverage.

  • What if I need help?

  • Is my information shared with my employer?

    All programs are confidential and in compliance with the Health Insurance Portability and Accountability Act (HIPPA). Any information shared with the SimplyWell team will not be disclosed, except in accordance with HIPPA laws. Your Protected Health Information (PHI) will not be shared with your employer.
